Registration Forms
You can complete the registration forms on the computer. Then, print them to bring with you on your initial visit. There are three Registration forms to complete:
BLC_RegForm_010121 (efill) This is three pages – The Registration, Child’s Health History and a Summary of BLC Policies. Each needs to be completed and signed.
The following forms (English or Spanish) need to be completed to obtain subsidies for your child to participate in BLC’s Food Program:
